Can I set up groups of notes in folders that are held on the phone and not on servers? Such as if I have nursing notes, can I separate them from my personal notes?

I think there is more than one answer to your question:
1) Notebooks allows you to create as many books as you want, and books can contain other books, too. So it is very easy to separate your private notes from job-related notes.
2) If you decide to synchronize Notebooks with a server, you can always choose which part of Notebooks hierarchy you want to sync; you can sync the whole contents, but you can also decide to sync just a specific part. - You do that by moving to the book you want to sync and then select "Sync - ..." from the context menu
3) Just to clarify: Notebooks never "holds" its documents on servers, they are always held locally. Notebooks uses servers for synchronization, which makes it very easy to get documents into and out of Notebooks, and also to have them available from your computer, too.
